COVID Proof of Vaccination or Weekly Negative Test Result Submission Portal

This portal is for all BRFV users (client business owners AND their employees) to submit proof of full COVID vaccination to BRFV management OR for unvaccinated individuals to submit weekly proof of a negative test result. Regardless of vaccination status or negative test result, all facility users at this time will still be required to be masked when in the facility. Please ensure that the masks you are wearing fit snugly against your face; masks should not be able to slip below your nose or mouth.
